Faith Formation Registration forms:

The below forms MUST be completed for children who will be attending Sacred Heart Faith Formation for the first time.

Parents of returning children will be required to review previously submitted forms to update information and renew medical form authorization.

This is intended to save returning parents from recompleting all the forms but protect our children by keeping all information up to date and accurate.


For purposes of information and safety/wellbeing, we need up to date forms for all youth in the program

Baptismal Certificate: Please Note: If you are registering a child at Sacred Heart for the first time, we will need a baptismal certificate. If the child was baptized at Sacred Heart, we will obtain the certificate from our records. If your child was baptized at another parish, YOU need to contact that parish and have a new copy of the certificate made by the parish for our records.

Prayers: Certain prayers are a part of our Catholic Faith Tradition. To help form our students spiritual life, we have developed grade level prayer goals. Each year builds on the prayers learned the year before. The full prayers can be found in the Faith Formation Handbook. As part of their own spiritual life and shared prayer time with their children, we encourage parents to teach/pray the prayers that are part of your child's goal with him or her.

Activity Release Form: If your child goes on a trip off parish property as part of the Faith Formation Program we need the following release form completed: Diocese of Gary Activity Release Form
